Madhouse (1981) - The Sister Who Has It All And The Sister Who Does Not...

For my birthday, I just want to get away with murder...

Julia is beautiful. Mary is horrifically disfigured from a fatal skin disease. Julia is an accomplished school teacher for the deaf. Mary is cooped up in a mental institution while Julia is ever so hesitant to visit her but does after Uncle James, the local priest, pleads with her to. Julia has a handsome boyfriend, Mary has a ferocious but loyal Rottweiler who obeys her every command, especially the command to kill. Julia is traumatized by her childhood memories of the evil things Mary did to her.

As the birthday approaches for the twins who look nothing alike, it will be a birthday party that nobody will ever forget. Mary escapes from the mental institution and her and the loyal Rottie go on a heinous killing spree. The higher the body count, the more RSVP's for their birthday party. Let's revise the guest list:

Sasha, one of Julia's students - killed by Mary's Rottie
Mr. Kimura - slaughtered by Mary
Helen - throat torn open by the Rottie

Wait, there is more. Uncle James has another guest in mind, Amantha Beauregard, the local parishioner, who is onto James when she finds out that she accidentally helped him carry a corpse for Julia's "surprise birthday party". Uncle James the Priest, then stabs her to death. I guess it's nice to know that horrific disfigured Mary has someone on her side. 

Uncle James meets Mary after work and blindfolds her to her surprise birthday party. Mary berates Julia only to get murdered by James. OK, spoke too soon about him being on Mary's side, this priest is just a psycho. Sam, Julia's boyfriend, returns back home after his taxi ride to the airport gets a flat tire, and gets attacked by the Rottie. He fights off the canine and drills a hole into it's head. He rescues Julia, who then hatchets her Uncle James to death.
