Dr. Rose Cotter works in a very rewarding yet emotionally trying profession as a Mental Health Psychiatrist. However, when her patient, a PHD student, describes how she witnessed her own professor take his own life, and now is seeing something extremely evil trying to get her, it keeps smiling at her, the patient convulses. As Rose calls for immediate help, she turns around and sees her patient standing right in front of her. She is smiling. Then she proceeds to slit her own throat with a shard from a broken vase...
And it continues from there on out. The smiling trend. Rose sees the same exact thing over and over again. Another patient from the unit, Carl, begins to smile at her taunting her by repeating over and over that she is going to die. The trauma pursues Rose, effecting her relationship with her fiance' Trevor, her sister and her family, when at her nephews birthday party, he opens up Rose's gift and to his shock and terror finds Rose's dead cat in the box. Then Rose sees her deceased patient, smiling at her...Rose has a severe breakdown..
When Rose was younger, she had found her own abusive mother's dead body after an overdose. She never quite came to terms with it by seeking the proper psychiatric care. However, she chose to be in the profession where it is very imperative that the Doctor maintain self control at all costs in order to be a benefit to psychiatric patients. If the Doctor loses self control, who will help these patients now? Rose realizes now that she is condemned.
Now it is time for detective work. Rose learns that her patient's professor was smiling at her as he took his own life, so she pays his wife a visit who claims he was acting very strange after he himself witnessed a woman commit suicide. Rose enlists in her ex Joel's help because he was the police officer who responded to her first patient's death. The cases they research all point to a chain reaction where someone experiences evil encounters that only they themselves can witness, before taking their own life. Then they pass the curse onto the only witness who sees them commit suicide. The cursed victims do not survive longer than a week. Rose was on her fourth day. However, there was one survivor who was able to dodge the curse, Robert Talley who is in prison for murdering a stranger. When they visit Talley in prison, he explains to them that the only way to break the chain is to kill someone else, in front of a witness that the curse will transfer onto.
During Rose's therapy session, Dr. Madeline Northcott begins to taunt her by smiling at her, reminding her that her time is coming. Later on Rose envisions murdering Carl while he is still smiling at her, and her supervisor witnesses it and while releasing a harrowing scream, rips his own face off. After her supervisor finds a knife in her car and calls the police, Rose drives away to her old abandoned family home. She realizes now it is time to face the demon. Her "mother" reminds Rose that she actually found her mother before she overdosed and left her to die. Her "mother" begins to morph into an oversized demonic version of her former self and Rose sets it on fire. The curse ends, or at least Rose thinks..
When she goes back to Joel's apartment, for comfort, he lets her know he will never leave her. Joel begins to smile. It turns out Rose is still at her old family home, and what she experienced was a hallucination. The demon rips off its face revealing a Russian Nesting Doll of grins and crawls into her mouth taking over her. Joel arrives to the house and sees Rose. She is smiling at him. She pours gasoline over herself and sets herself on fire, right in front of Joel.
The curse continues...
